About Iconic Care
Iconic Care is a trusted and compassionate provider of support services for individuals in need of assistance through the National Disability Insurance Scheme (NDIS) and Home Care Packages (HCP). Our primary focus is to empower individuals with disabilities, the elderly, and those with special care needs, enabling them to live a life of dignity, independence, and fulfillment within their own homes and communities.
NDIS Services
As an NDIS-registered provider, Iconic Care offers a wide range of support services that are tailored to meet the unique needs of NDIS participants. Our team of dedicated professionals is committed to delivering high-quality care and assistance that aligns with the goals and aspirations of each participant. Our NDIS services include:
- Therapeutic Supports: Providing access to allied health professionals, such as Occupational Therapists, Speech pathologists, Psychologists, Exercise physiologists, and physiotherapists.
- Personal Care: Assistance with daily living activities such as bathing, dressing, and grooming.
- Community Access and Participation: Supporting participants to engage in community activities, social interactions, and recreational pursuits.
- Support Coordination: Helping participants to understand and implement their NDIS plans, coordinating various supports and services.
· Supported Independent Living (SIL) & Specialist Disability Accommodation (SDA):
SDA: we offer housing and 24/7 assistance for people who require specialist housing and cannot live independently. SDA is funded separately from SIL through the NDIS
SIL: We offer in-home support services to help people with disabilities live as independently as possible. Iconic care supply staff to help with daily activities like cooking, cleaning, and personal grooming. SIL is funded through the individual’s NDIS package

Iconic Care is partnering with Trilogy Care to ensure you have access to high-quality home care service via Home Care Package.
Trilogy Care
Trilogy Care supports you to live well at home longer by helping you build and self-manage your Home Care Package. Trilogy Care is an approved Home Care Package provider and is registered with My Age Care to deliver Home Care Package support.

Our goal is to support your independence and well-being, allowing you to live comfortably at home. Our Home Care Packages are designed to provide the right level of support, from basic care needs to more comprehensive assistance.
Our HCP services include:
In-Home Support: Assistance with household tasks, meal preparation, and general home maintenance. Personal Care and Wellbeing: Helping with mobility, medication management, and personal hygiene. Nursing and Allied Health Services: Access to registered nurses and healthcare professionals for specialized care. Social Support: Encouraging engagement with social activities and maintaining a connection with the community. Transport: Assistance with travel to appointments or community events.
